Reschedule and cancellation fee
If you cancel or reschedule less than 48 hours before the appointment, the reschedule fee is $500 for a disclosure polygraph, $200 for a maintenance polygraph, and $200 for attorney clients.

Forgot your password
Use Forgot password to email yourself a reset link. The link expires and works once. If the email does not arrive, check your spam or junk folder.
Editing a questionnaire
You can edit your questionnaire until the appointment. At the appointment time, answers lock so the exam record stays as it was. If you need a change after that, send a request below and the office will help.
What a partner or therapist can see
A partner or therapist can see a shared questionnaire only after the agreements are signed and the client shares it with them. Until then, those pages stay private.
An email did not arrive
Check the spam or junk folder, and add the office email address to your contacts. You can also send a request below and choose “Email not received.”
